You must have a Wisconsin instructional permit or an International license in order to begin driving lessons with us. Adult DMV Permit Requirements: Provide proof of U.S. citizenship, legal permanent resident status, conditional resident status or temporary visitor status Provide proof of name and date of birth Provide proof of identity Provide proof of Wisconsin residency if you are 18 years of age or older Provide your Social Security number (you can fulfill your proof of identity requirement by presenting your Social Security card) $35.00 cash or check. 4 Lakes Driving Lesson Scheduling You must have your permit prior to taking a driving lesson. Our instructors are available 7 days a week throughout the day. Driving lessons must be cancelled at least 24 hours in advance. Saturday, Sunday, and Monday appointments must be cancelled by Friday by 3:00pm. If the lesson is not cancelled, you will be charged. Please call the office for more information.
